RESPONSIBILITIES:
1. Resident Care
• Administer nursing care to residents based on their individualized care plans, ensuring attention to their physical, emotional, and mental health needs.
• Monitor and assess resident health conditions, including vital signs, and promptly report changes or concerns.
• Coordinate care with doctors, physical therapists, and wellness coordinators, nutritionists to ensure holistic care for each resident.
• Ensure the daily living activities, such as bathing, dressing, and hygiene, are appropriately provided.
• Educate residents and their families on health conditions, medication, lifestyle changes, and care plans to promote understanding and cooperation.
Nursing Protocols:
• Administer medications as prescribed by the DMO, including oral, topical, and injectable medications. Monitor for side effects or adverse reactions to medications and report any concerns to the physician immediately.
• Maintain accurate medication records and clinical documents in accordance with the SOP.
• Respond promptly to medical emergencies and emergency protocols, such as CPR and first aid, and ensure that all necessary emergency equipment and crash cart is readily available and functional.
• Implement infection prevention and control practices, such as proper hand hygiene, sterilization of equipment, and isolation protocols when necessary.
• Ensure the environment is safe and clean, reporting any safety hazards and assisting with implementing corrective actions.
• Ensure the caretakers are aware of the resident health conditions and coach them to provide appropriate care. Monitor care service quality and adherence to SOPs by routine floor rounds
• Ensure face id attendance is marked/ duty rosters are adhered and inputted in the AL App. Implement disciplinary practices, guide and counsel team members for adherence.
